This patient was referred to me for a #rootcanaltreament of an upper right second molar (17) under the suspicion that the tooth could be presenting a perforation which was confirmed after the inspection with the surgical microscope. My advice for those who face this problem or accident is not to panic as perforations can be repaired with the use of the right technique.

This perforation was repaired using a bioceramic material and the treatment was completed in a second session. The result may vary from patient to patient but it’s very predictable in the vast majority of cases. Let me save your tooth.

This patient was referred to me for a re-treatment of an upper left molar (27). This tooth had a broken Gates Glidden on the apical third of the MB root. With the use of the ultrasonic, the surgical microscope and a lot of patience I managed to retrieve the broken instrument. This tooth was treated 2 times before me and I hope that I could extent its life for many more years. Extraction should be only advised when the tooth can’t be saved. The result may vary from patient to patient but it’s very predictable in the vast majority of cases. Let me save your tooth.

Double curvatures or “S” shape type canals are always a challenge due the the high risk of file separation. One of the most important things is to warn the patient about this possibility before the treatment and for that a proper assessment and diagnosis of the situation is essential.

In this case the double curvature of the mesial canals of the second lower molar was successfully shaped. The secret is to use the right instruments and to increase the size and the taper of the files step by step, without big jumps and recapitulating and irrigating all the time making sure there is a nice glide path. The result may vary from patient to patient but it’s very predictable in the vast majority of cases.

This tooth was presenting a big apical periodontitis surrounding the mesial root. I complete 90% of the cases I do in one single appointment, the use of a proper irrigation protocol combined with the use of bioceramics and the surgical microscope make root canal treatments predictable and totally safe. This tooth healed perfectly after the treatment, creating new and healthy bone. Let me save your tooth, you are going to need it many years. This tooth was presenting a broken file in the MB canal. The difficulty about this case was the size of the broken instrument and the mild curvature of the MB canal making very difficult for this long broken file to “turn” through this curvature. I really prefer when the broken instruments are small because if you can reach them with the ultrasonic most probably they will come out. This case took me 2 appointments of 2 hours each in order to complete the whole process. The use of the surgical microscope is key as is very important to be accurate with the ultrasonic tips in order to not cause any perforation while retrieving the broken file.
