This case was referred to me for a root canal trea

This case was referred to me for a root canal treatment of a lower left central incisor (31).

The tooth was showing a chronic apical periodontitis, likely due to a previous trauma. As a result, a calcific metamorphosis, or canal calcification, was present.

The complexity of the case lay in the calcification of the canal in a very narrow root, making the treatment challenging.

Since half of the canal was calcified, the freehand approach carried significant risks. Even a slight deviation in the trajectory during the access could lead to a perforation. Therefore, a guided endodontic access was planned.

To create the guide, I used information from a CBCT (3D x-ray) and an intraoral scan (3D modeling of the teeth). Using specialized 3D modeling software, a digital model of the mouth was recreated. For that purpose, I visited @charismatic.dl lab and together we designed the guide.

With the aid of the guide, the root canal treatment was successfully completed, and the canal calcification was effectively managed.

This highly complex case was successfully resolved thanks to meticulous digital planning and the use of guided technology.

This case was referred to me for the retreatment of a lower incisor (#41). The tooth presented with a big chronic apical periodontitis, which seemed to be affecting the teeth next to it as well.

In this kind of situation, it is very important to do a meticulous diagnosis to identify the source of the problem so that we do not treat the other teeth mistakenly without reason.

The large size of the lesion is not a synonym for tooth extraction, which unfortunately we see quite often in the decision-making processes. In the absence of bacteria, the bone heals, and we can save the tooth.

The retreatment was carried out over two sessions with a three-week calcium hydroxide dressing between them.

This case was referred to me for a retreatment of the upper left second premolar (25). The tooth presented with chronic lateral periodontitis as a result of a previous perforation. The retreatment was performed, and the perforation was repaired using bioceramics. After six months, there was complete resolution of the chronic lateral periodontitis, allowing us to preserve the tooth for many more years.

This patient was referred to me for a #rerootcanaltreatment of an upper left second premolar (15). The tooth was presenting a couple of metal post and a silver point obturation. Within the always difficulty of re-root canal treatments, removal of metal posts and silver points are quite predictable with the proper skills. This tooth was already treated 2 times in the past so preserve as much natural tooth structure was very important. Every time that the tooth is root canal treated or re-treated adds another layer of difficulty for the next treatment making very challenging sometimes to fix what it was done 20 or 30 years ago. My advice is always to find the right specialist with experience to fix this kind of situations to avoid further damage.

My advice for those who face this problem or accident is not to panic as perforations can be repaired with the use of the right technique.

This patient was referred to me for a re-treatment of an upper left molar (27). This tooth had a broken Gates Glidden on the apical third of the MB root. With the use of the ultrasonic, the surgical microscope and a lot of patience I managed to retrieve the broken instrument. This tooth was treated 2 times before me and I hope that I could extent its life for many more years. Extraction should be only advised when the tooth can’t be saved. Double curvatures or “S” shape type canals are always a challenge due the the high risk of file separation. One of the most important things is to warn the patient about this possibility before the treatment and for that a proper assessment and diagnosis of the situation is essential.

In this case the double curvature of the mesial canals of the second lower molar was successfully shaped. The secret is to use the right instruments and to increase the size and the taper of the files step by step, without big jumps and recapitulating and irrigating all the time making sure there is a nice glide path. The result may vary from patient to patient but it’s very predictable in the vast majority of cases.

This tooth was presenting a big apical periodontitis surrounding the mesial root. I complete 90% of the cases I do in one single appointment, the use of a proper irrigation protocol combined with the use of bioceramics and the surgical microscope make root canal treatments predictable and totally safe.
