$This case was referred to me for the retreatment of a lower first molar (#46). The tooth presented with chronic apical periodontitis and a fractured instrument in the MB canal. For all my retreatments, the use of CBCT is an essential part of the planning process as it allows me to accurately measure the canals. In this case, it also helped to locate and assess the broken instrument, enabling me to approach the case with accurate information. The retreatment was carried out over two sessions. The first session was dedicated solely to removing the fractured instrument, which was quite difficult. It took me two hours of work to retrieve the file. This case was referred to me for a retreatment of the upper left second premolar (25). The tooth presented with chronic lateral periodontitis as a result of a previous perforation. The retreatment was performed, and the perforation was repaired using bioceramics. After six months, there was complete resolution of the chronic lateral periodontitis, allowing us to preserve the tooth for many more years.

$This case was referred to me for a #rerootcanaltreatment of an upper left second premolar (25). The tooth was presenting a lateral periodontitis which can indicate either a tooth fracture or an untreated lateral canal. For these kind of cases I always take a CBCT (3D x-ray) to ensure proper treatment planing and to obtain more information about the possible cause of the problem. I like to be very honest with all my patients and I always explain them the challenges of every case and the possible outcomes. The treatment was successful and, although a lateral canal wasn’t visible on the x-ray, it was likely the cause of the problem. A proper disinfection protocol and the use of advanced technologies and techniques are crucial for improving treatment success rates.
