This case was referred to me for a retreatment of the upper left second premolar (25). The tooth presented with chronic lateral periodontitis as a result of a previous perforation. The retreatment was performed, and the perforation was repaired using bioceramics. After six months, there was complete resolution of the chronic lateral periodontitis, allowing us to preserve the tooth for many more years.

For these kind of cases I always take a CBCT (3D x-ray) to ensure proper treatment planing and to obtain more information about the possible cause of the problem. The treatment was successful and, although a lateral canal wasn’t visible on the x-ray, it was likely the cause of the problem.
