  • The case of today is a RE RCT of a lower right first molar (#46) with a perforation in the last third of the mesial root. When do we have to extract the tooth or try to save it? Well, I don’t think there is a straight answer for extreme situations like this but in this case after explaining the difficulties and risks, the patient was keen to try to save the tooth. This tooth presented and apical periodontitis in the last third of the mesial root caused by a perforation done on the previous treatment. To fill the canals I used only Bioceramic sealer without guttapercha but using hydraulic force to spread the sealer to every corner. The result is a complete healing of the apical periodontitis.   • Endodontics as science and root canal treatments, if done properly, WORKS. In this case I did a RE root canal treatment of an upper right second molar (#17). For those who are not familiar with dentistry, that black “bubble” surrounding the tip of the root is an apical periodontitis (infection caused by bacteria inside of the tooth). Re root canal treatments are always a challenge, to re-do the treatment its complicated but with the right techniques knowledge and equipment I can save a tooth doomed to extraction.   • Lateral apical periodontitis usually means two things, a root fracture or a lateral canal as in this case. In order to be able to fill this kind of lateral canals an excellent irrigation protocol is key. On top of constant irrigation during the whole treatment, part of my final irrigation protocol is the use of passive ultrasonic irrigation (PUI). The use of this technique allows me to clear these lateral canals in order to be filled with Bioceramic sealer during the hydraulic condensation phase.
