  • STAY FOCUSED. Only a 5% of the upper first premolars present a 3rd canal. It’s very important to know that rare anatomies can occur and that we could find them in our daily practice. To have an excellent knowledge of the tooth anatomy is key to not make cases like this a problem. As well, using the surgical microscope it’s the only way where I can identify and treat the bifurcation of the canal in the middle third of it.   • Only with the use of my microscope and the last high-end bioceramic sealers I’m able to find and fix accessories structures and missing canals, sometimes they are very small but to treat them is VERY important. This is a case of a re-treatment of an upper second premolar with an apical periodontitis (infection) caused but an improper RCT. Thanks to the use of the CBCT (mandatory for all my RE RCTs) I can analyze which is the real problem and make a proper treatment analyzing the tooth millimeter by millimeter. What you see on the image is a complete healing of the bone after my treatment.   • This is a case of a re-treatment of an upper second premolar with an old silver point causing an infection. Silver points were used many years ago as an obturation material. To remove this, it’s very important to use the surgical microscope as the silver points can be fragile and tend to fracture. As well, it’s v to see all the little corners inside of the tooth that has to be cleaned this can be only fixed that with the naked eye would be totally impossible.   • Only by using my surgical microscope I’m able to identify and treat these small anatomical structures. This MB2 canal (that’s the name 😁), is present in more than 60% of the upper first molars. It’s super tiny but as important as the main ones.
